This information is necessary to book an appointment and collect payment. In addition, you will be required to complete a medical history intake form with a specific set of medical discourses we require to share with you specific to the jurisdiction in which we operate in. During this intake process, we will also share a comprehensive notice of medical privacy practices for information The Wellness Hub collects within your treatment session. The medical privacy practice information provided is a choice, as it is not required to book a treatment or interact with any of our digital platforms.

With your permission, The Wellness Hub may send you emails about healthcare information, appointment details and reminders and clinic updates. you can choose to subscribe or unsubscribe to reminders, email notifications and marketing emails. The Wellness Hub may share your information within The Wellness Hub company to better serve you and provide you with comprehensive care.

When you contact The Wellness Hub via email or phone, we will ask for information to identify you on Jane which is needed to provide timely assistance. In addition, we may also ask for your feedback in communicating with us to improve our services.

When you book an appointment, receive treatment, or purchase something from The Wellness Hub, as part of the buying and selling process, we collect personal information that you give us including your name, address and email address.
